Also, uncheck the boxes for hiding known file extensions and hiding protected operating system files. We want to see it all. When we finish here, it would be a good idea to rehide the protected operating system files but leave the rest to be shown.

Reboot in normal mode.

I followed your instructions but have a few problems.
First when i try to fix "O20 - Applnit_DLLs: mad.dll" in hijackthis, it still stays there.
2nd i deleted C:\WINDOWS\System32\prvtect.exe and C:\PROGRA~1\COMMON~1\WinTools\ last time, but obviously it was still there as you told me to delete it again. When i tried to find them (even with hidden files showing) niether of them were there.
And thirdly C:\WINDOWS\System32\mad.dll i could find, but whilst in safe mode with nothing else running, it would not let me delete it as it said it was being used by another program.

Sorry about my torubles, i just letting you know what happened when i did as you asked. Anyway here is my log after all this.

I caould not be sure the files had been deleted so when we removed the registry entries, I also asked for file deletion as a double check. Most of it gone but the O20 entry is being persistant.

First:
Download Killbox from here: http://download.broadbandmedic.com/killbox.exe

Run Killbox.exe and be sure that 'Delete on Reboot is checked'

Select the Folder Icon to the Right of the right of the Address Area and find the following file(s) one at a time:
C:\Windows\System32\mad.dll

Note: You can also cut and paste the files listed above

After each one, press the delete button on the far right of the address bar

Each time you will get a dialog box asking if you want to reboot
Answer yes since we are only dong one file
Your computer will reboot and delete the file

Verify that all the file has actually been deleted.


Second:
Before we begin, please be sure that HiJackThis is in its own folder. This will allow us to use backups to restore entries if necessary. Please do not put HiJackThis in a temporary folder, or on the Desktop. I suggest using 'c:\program files\hijackthis\' or C:\HiJackThis\, but any name you choose is fine.

Reboot in Safe Mode* and run HiJackThis. <-- IMPORTANT

Check the following items in HijackThis.

Were you able to delete the mad.dll file uisng Killbox??

PLease check and let me know if the file exists. It should be located in c:\windows\system32\. If not there, then try c:\windows\.

Download FindnFix.exe from here:

Double Click on the FindnFix.exe and it will install the batch file in its own folder.

Open the FindnFix folder and double click on !LOG!.bat
